CubFest 2004

Registration Form

The purpose of this registration form is to assist us in allowing for sufficient RV spaces, number of Johnny's and reasonable estimates for meal purposes, to avoid either over or under estimating for quantities. This will allow us to get better prices for meal rates.
Also, we would like to be able to offer hats and t-shirts as memorabilia of our first CubFest to those who wish to purchase these items. To accomplish this requires that we pre-order them so they will be available during CubFest.
Note: This is totally non-profit and the prices for meals, hats and t-shirts will be our actual costs and not a penny more. We wish to provide the best of service so that your visit with us will be enjoyable and memorable!.
